Малоинвазивная диагностика случайно обнаруженных кистозных процессов поджелудочной железы

Аннотация

Кисты поджелудочной железы (КП) часто обнаруживаются случайно при ультразвуковом исследовании. Клинические проявления не выражены. Однако КП имеют риск злокачественности.
В данном обзоре рассмотрены критерии для клинической диагностики КП, роль трансабдоминальной ультрасонографии, включая УЗИ с контрастным усилением, эндоскопического ультразвукового исследования (эUS) и КТ.
эUS имеет наибольшее значение в дифференциальной диагностике и прогностической характеристике случайно выявленной КП. В одном исследовании можно выполнить УЗИ с высоким разрешением, эUS, а также тонкоигольную аспирацию содержимого кисты, стенки кисты и её солидных компонентов. Критерии высокого риска при оценке муцинозных кист поджелудочной железы, в основном, базируются на результатах эUS и КТ. Тем не менее, несмотря на диагностический прогресс и улучшение визуализационных параметров, дифференциальная диагностика и определение тактики лечения остаются сложными. В этом обзоре будут обсуждаться проблемы и подходы к диагностике случайно выявленных КП.
